This form is required to be submitted within the first 2 weeks of the semester that you are enrolled in. If you are not enrolled in the semester or requesting for a future semester please proceed. View the Leaves of Absence policy for more information. Please note that you will be dis-enrolled from any classes that you are enrolled in for the upcoming semester.
